This session details how to develop an on-going healthcare improvement plan and the process results necessary for becoming a vital partner in today’s ACO’s, Bundled Care Payment Initiatives, Medicare Shared Savings Programs and Managed Care Organizations. These partnerships have to be sought, developed and sustained. The partnership requires both quality and financial results to be delivered in a synchronized approach.

Objectives:
At the completion of this session, the participants will be able to:

1. Pursue and advance partnership opportunities with Hospital/Health Care Systems
2. Develop a systematic measurement of success for the required quality and financial results to fulfill the partnership agreement
3. Establish a strategic plan focused on quality and financial results to achieve success and position the organization for future involvement in episodic payment and shared risk agreements
4. Develop enhanced quality and financial results that would benefit the Hospital, Health System, Managed Care Organization and Managed Medicaid Care network
